Patient and Appointment Info

Your First Appointment With Us:

Once you or your referring doctor has made your appointment with us, we will send you an information packet. This packet will include directions to our office, a New Patient Information Sheet, and a sheet to help you list all the medication you are currently taking. Please fill out this paper work prior to coming in for your appointment.

 

What to Bring to Your Appointment: :

  • Completed New Patient Information Sheet
  • A list of all your medications
  • Any eye drops you may be currently using
  • Your medical insurance cards and information
  • Drivers license or ID card
  • Your eyeglasses and/or contacts
  • Referral form from your primary care physician - if required

 

About Your Appointment:

Your first appointment with us may take approximately two hours. Your eyes may be dilated for this initial visit. If you know you are sensitive to dilation, you may want to bring someone with you to drive you home after your exam. It will help us to help you if you are prepared to answer some of the following questions:

  • What brings you to our office today (my doctor told me to see you because ….., or I’m having a problem with ……….)
  • How long have you been experiencing this problem?
  • Are you having any pain? If yes, how often? How long does it last?

 

CECA SURGERY SCHEDULING :

Dr. Michael performs surgery at the Stony Point Surgery Center, located at 8700 Stony Point Pkwy, off of Chippenham Pkwy, a map will be provided for you.

Our standard surgery days are Tuesdays afternoon and Thursdays all day.

The day of your consultation, if surgery is indicated and desired by you, you will meet with our surgical coordinator at the end of your visit. She will schedule your surgery date and time.

You will be given a surgery packet that will include all the information you will need to prepare for your eye surgery.

On the day of your surgery, you will need to have someone with you to drive you home. You should expect to be at the surgery center approximately two hours that day.

We will see you back in our office the day after your surgery for a post op visit. The time and location for your post op visit will be scheduled at the same time you schedule your surgery.
